Greetings from Trinity Foundation. We believe that advancing health care transforms individuals and communities. In fact, research shows that “thriving” communities are ones that have expanded their reach beyond their own family unit. Supporting Trinity Foundation is one way you can stretch beyond your family unit.

Trinity Foundation and UnityPoint Health – Fort Dodge work hard every day to expand our reach and improve the health of our community. Our focus and energy are centered around our passion for taking care of our community and being your trusted provider. Patient care at UnityPoint Health – Fort Dodge is enhanced because of your investment and commitment.

Trinity Foundation works hand in hand with UnityPoint Health – Fort Dodge leaders to ensure your gifts have the greatest impact on our organization. We work to support the areas that best serve our community and region and keep care locally. We want to take this opportunity to tell you how your gifts have made an impact. The list is impressive, so we’ll just highlight a few items we have purchased on your behalf.

The Norma Schmoker Cancer Center provides comprehensive state-of-the-art care to patients with cancer. In addition to medical and radiation oncology, we provide care coordination and support for individuals living with cancer. Each year, Trinity Foundation provides funding for wigs, transportation and daily living necessities, as well as equipment that is vital to providing this care locally. We are currently adding two exam rooms to address the increased oncology volume and creating a therapy room to provide physical and occupational therapy to patients with cancer. These projects are funded with donor dollars and support the need for local cancer care.

Trinity Foundation has also purchased equipment for the surgery department that will improve patient outcomes and patient wait times, as well as increase staff efficiency. Some of the items purchased include: two De Mayo Ortho Positioners which secure the knee and prevent movement during a knee replacement; a Nathanson Flex System Retractor, used during laparoscopic procedures to allow a better view of the internal organs; and additional Urology Semi-Rigid Cysto Trays, enabling the surgery team to increase the number of urology surgical cases performed each day.

When you visit our organization, you may notice a variety of carts and beds. These carts and beds are designed for specific purposes and keep patients and staff safe. We recently purchased a second cataract patient surgery cart that adjusts the patient to provide comfort and best positioning while under the microscope, and also increases safety and efficiency for patients and staff.

We have also purchased patient transport carts for surgery and the cardiac catheterization lab that improve patient comfort and staff safety. These carts are equipped with motorized drive, decreasing the need for transport staff while helping to reduce caregivers’ back stress.

Safety is clearly a top priority and another reason why we purchased an adult changing table for our new outpatient rehab services area. This changing table is located at Entrance C and provides a safe environment for both the patient and caregiver to easily transfer and move a patient to address their needs in the bathroom.

Lastly, we have purchased two Hoyer lifts. Not everyone is capable of transferring to a bed or cart. In that case, we have Hoyer lifts throughout our facility that assist with the transfer, keeping patients safe and reducing workplace injuries for staff.

Please know your gifts have an impact. This is a quick snapshot of what Trinity Foundation has accomplished in the past year with your help. In 2022, Trinity Foundation plans to transfer at least $1.8 million to UnityPoint Health – Fort Dodge. This long legacy of providing needed transportation, medications, medical equipment and health care services is something you can be proud of. A donor advised fund is an account that you set up but which is managed by a nonprofit organization. You contribute to the account, which grows tax-free. You can recommend how much (and how often) you want to distribute money from that fund to Trinity Foundation or other charities. You cannot direct the gifts.

An endowed gift can create a new endowment or add to an existing endowment. The principal of the endowment is invested and a portion of the principal’s earnings are used each year to support our mission.

Tax on the growth in value of an asset—such as real estate or stock—since its original purchase.

Securities, real estate or any other property having a fair market value greater than its original purchase price.

Real estate can be a personal residence, vacation home, timeshare property, farm, commercial property or undeveloped land.

A charitable remainder trust provides you or other named individuals income each year for life or a period not exceeding 20 years from assets you give to the trust you create.

You give assets to a trust that pays our organization set payments for a number of years, which you choose. The longer the length of time, the better the potential tax savings to you. When the term is up, the remaining trust assets go to you, your family or other beneficiaries you select. This is an excellent way to transfer property to family members at a minimal cost.

You fund this type of trust with cash or appreciated assets—and may qualify for a federal income tax charitable deduction when you itemize. You can also make additional gifts; each one also qualifies for a tax deduction. The trust pays you, each year, a variable amount based on a fixed percentage of the fair market value of the trust assets. When the trust terminates, the remaining principal goes to Trinity Foundation as a lump sum.

You fund this trust with cash or appreciated assets—and may qualify for a federal income tax charitable deduction when you itemize. Each year the trust pays you or another named individual the same dollar amount you choose at the start. When the trust terminates, the remaining principal goes to Trinity Foundation as a lump sum.

A beneficiary designation clearly identifies how specific assets will be distributed after your death.

A charitable gift annuity involves a simple contract between you and Trinity Foundation where you agree to make a gift to Trinity Foundation and we, in return, agree to pay you (and someone else, if you choose) a fixed amount each year for the rest of your life.
