Moving Forward to Serve Our Community

The past months have been a struggle for everyone. COVID-19 has changed our plans and has us all looking at life a bit differently. However, amid all the stress and anxiety of a pandemic, UnityPoint Health – Fort Dodge has also managed to move forward on some strategic initiatives that we are very excited about. Listed below are some great changes that will help our organization grow and serve our community.

  • In August, pediatric hospitalists from Blank Children's Hospital began providing onsite coverage at Trinity Regional Medical Center for inpatient pediatrics and newborn nursery. Pediatric hospitalists are now available and will help keep patients local.
  • We are proud to announce our new eastside location for UnityPoint Clinic Express. The new location will be in the new mall development area, Corridor Plaza. This prime location will be at the corner of First Avenue South and South 29th Street. The project team is excited to get construction underway as well as to "flip" our current Walk-In Clinic to this convenient Express model of service.
  • We are pleased to announce that outpatient neurology services will be available to our community starting this fall. Based on our successful partnership with Center for Neurology, Orthopedics & Spine (CNOS) for orthopedic care, UnityPoint Health is excited to expand our partnership with CNOS to provide local neurology care. CNOS neurologist Dr. Andrews will begin seeing patients in Fort Dodge two times a month, with additional advance practitioner coverage as the clinic grows.
  • UnityPoint Health – Fort Dodge welcomed orthopedic surgeon Dr. Anand in June, and now we officially have full-time orthopedic coverage back in Fort Dodge. Dr. Anand has hit the ground running and is doing both inpatient and outpatient surgical cases. For the month of June, orthopedic surgical cases at Trinity Regional Medical Center were at 126 percent of budgeted volumes!
  • Berryhill Center launched our Mobile Crisis Response Team on July 1. The Mobile Crisis Response Team provides on-site, face-to-face mental health services for an individual or family experiencing a mental health crisis. The team of professionals can respond wherever the crisis is occurring in our geographical area—in an individual's home, the community or other locations where an individual lives, works, attends school or socializes. Within 24 hours of completion of the crisis assessment, a case manager will place a phone call to follow up with the individual. To access the Mobile Crisis Response Team, a neighbor, parent, teacher, law enforcement or concerned individual can call Your Life Iowa at (855) 581-8111.
  • Behavioral Health Urgent Care at Berryhill Center is now open Monday through Friday, 10:00 a.m. to 8:00 p.m., located on the lower east entrance of the building. The new behavioral health urgent care clinic provides services to individuals who are needing mental health services, psychiatric evaluation and assessment, addiction medicine, crisis services and community resources. Since opening the service in early April, we have served over 54 patients. No referral or appointment is needed. Both pediatric and adult patients are welcome.

We hope you find these organizational developments as exciting as we do. Thank you for all you have done to keep us moving forward during these uncertain times. We are in this together, and together we will get through this. Thank you for standing with us; you are at the center of everything we do. You Matter!

Sincerely,

Shannon McQuillen

an individual or organization designated to receive benefits or funds under a will or other contract, such as an insurance policy, trust or retirement plan

"I give to Trinity Foundation, a nonprofit corporation currently located at 802 Kenyon Road, Fort Dodge, IA 50501, or its successor thereto, ______________ [written amount or percentage of the estate or description of property] for its unrestricted use and purpose."

able to be changed or cancelled

A revocable living trust is set up during your lifetime and can be revoked at any time before death. They allow assets held in the trust to pass directly to beneficiaries without probate court proceedings and can also reduce federal estate taxes.

cannot be changed or cancelled

tax on gifts generally paid by the person making the gift rather than the recipient

the original value of an asset, such as stock, before its appreciation or depreciation

the growth in value of an asset like stock or real estate since the original purchase

the price a willing buyer and willing seller can agree on

The person receiving the gift annuity payments.

A donor advised fund is an account that you set up but which is managed by a nonprofit organization. You contribute to the account, which grows tax-free. You can recommend how much (and how often) you want to distribute money from that fund to Trinity Foundation or other charities. You cannot direct the gifts.

An endowed gift can create a new endowment or add to an existing endowment. The principal of the endowment is invested and a portion of the principal’s earnings are used each year to support our mission.

Tax on the growth in value of an asset—such as real estate or stock—since its original purchase.

Securities, real estate or any other property having a fair market value greater than its original purchase price.

Real estate can be a personal residence, vacation home, timeshare property, farm, commercial property or undeveloped land.

A charitable remainder trust provides you or other named individuals income each year for life or a period not exceeding 20 years from assets you give to the trust you create.

You give assets to a trust that pays our organization set payments for a number of years, which you choose. The longer the length of time, the better the potential tax savings to you. When the term is up, the remaining trust assets go to you, your family or other beneficiaries you select. This is an excellent way to transfer property to family members at a minimal cost.

You fund this type of trust with cash or appreciated assets—and may qualify for a federal income tax charitable deduction when you itemize. You can also make additional gifts; each one also qualifies for a tax deduction. The trust pays you, each year, a variable amount based on a fixed percentage of the fair market value of the trust assets. When the trust terminates, the remaining principal goes to Trinity Foundation as a lump sum.

You fund this trust with cash or appreciated assets—and may qualify for a federal income tax charitable deduction when you itemize. Each year the trust pays you or another named individual the same dollar amount you choose at the start. When the trust terminates, the remaining principal goes to Trinity Foundation as a lump sum.

A beneficiary designation clearly identifies how specific assets will be distributed after your death.

A charitable gift annuity involves a simple contract between you and Trinity Foundation where you agree to make a gift to Trinity Foundation and we, in return, agree to pay you (and someone else, if you choose) a fixed amount each year for the rest of your life.
