UnityPoint Health - Fort Dodge Receives Circle of Life Award

UnityPoint Health was recently named a Circle of Life Award winner in recognition for its innovative palliative care program that improves the care of patients with serious or life-limiting conditions. More important, the program helps patients and families navigate the health care system when they are struggling with the impact of complex and serious illness. The Circle of Life award celebrates innovative programs across the nation that has made great strides in palliative and end-of-life care. This is the 14th year for the Circle of Life award.

"UnityPoint Health is proud to receive the Circle of Life award in recognition of innovation in palliative care," remarks CEO Sue Thompson. "Providing the type of care, comfort and support that helps seriously ill people achieve their best possible quality of life is important and noble work, and we applaud our palliative care providers and team members for their commitment to excellence."

UnityPoint Health has successfully developed an integrated palliative care program across its multi-regional health system in Iowa and Illinois. Parts of its approach to chronic care management, palliative care ensures access to services for patients with serious illnesses and is provided across all care settings, from the hospital to home. The palliative care team works together to follow the patient and coordinates services in close collaboration with the patient's primary care provider and specialists.

"By creating a unified palliative care program, we are better able to coordinate care at the right time and in the right place, relieving the symptoms and stress caused by a serious illness and helping patients and families achieve their goals of care," states Lori Bishop, Executive Director of palliative care and hospice services at UnityPoint Health. "This system approach has improved patient outcomes and the overall care experience."

The UnityPoint Health palliative care program was chosen by a selection committee made up of leaders from medicine, nursing, social work, ethics and health administration.

A donor advised fund is an account that you set up but which is managed by a nonprofit organization. You contribute to the account, which grows tax-free. An endowed gift can create a new endowment or add to an existing endowment. The principal of the endowment is invested and a portion of the principal’s earnings are used each year to support our mission.

A charitable remainder trust provides you or other named individuals income each year for life or a period not exceeding 20 years from assets you give to the trust you create.

You give assets to a trust that pays our organization set payments for a number of years, which you choose. The longer the length of time, the better the potential tax savings to you. When the term is up, the remaining trust assets go to you, your family or other beneficiaries you select. This is an excellent way to transfer property to family members at a minimal cost.

You fund this type of trust with cash or appreciated assets—and may qualify for a federal income tax charitable deduction when you itemize. You can also make additional gifts; each one also qualifies for a tax deduction. The trust pays you, each year, a variable amount based on a fixed percentage of the fair market value of the trust assets. When the trust terminates, the remaining principal goes to Trinity Foundation as a lump sum.

A beneficiary designation clearly identifies how specific assets will be distributed after your death.

A charitable gift annuity involves a simple contract between you and Trinity Foundation where you agree to make a gift to Trinity Foundation and we, in return, agree to pay you (and someone else, if you choose) a fixed amount each year for the rest of your life.
